HIPAA Notice of Privacy Practices

This notice describes how medical information about you may be used and disclosed, and how you can get access to that information. Please review it carefully.

Our duties

KAM Extending Hands LLC is required by law to maintain the privacy and security of your protected health information, to give you this notice of our legal duties and privacy practices, to notify you if a breach occurs that may have compromised your information, and to follow the terms of the notice currently in effect.

This notice is effective as of August 11, 2026, and applies to all protected health information we hold. It is separate from our website Privacy Policy, which covers information collected through this website.

How we may use and disclose your health information

Treatment. We may use and share your health information to provide and coordinate care, for example within your child's interdisciplinary team so behavioral and home health services work from one plan.

Payment. We may use and share your health information to bill and obtain payment, for example when we verify benefits or request authorization from your insurance plan.

Health care operations. We may use and share your health information to run our practice, improve quality, and train staff.

We may also use or disclose your information where the law requires or permits it, for example for public health activities, health oversight, to prevent a serious threat to health or safety, or in response to a lawful order.

Uses that require your written authorization

Most uses and disclosures not described in this notice will be made only with your written authorization, including most uses for marketing purposes and any sale of your information. You may revoke an authorization in writing at any time, and we will stop the future uses it covered.

Your rights

You have the right to inspect and receive a copy of your health records, to request corrections, to request restrictions on certain uses and disclosures, to request confidential communication by other means or at another location, to receive an accounting of certain disclosures, and to receive a paper copy of this notice on request.

To exercise any of these rights, contact our Privacy Officer using the details below. We will respond within the timeframes the law provides.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services, Office for Civil Rights, at hhs.gov/ocr or 1-800-368-1019. You will never be penalized or retaliated against for filing a complaint.
